Please remember that insurance reimbursement is a contract between you and your insurance carrier. Know and understand your rights and responsibilities. Your co-pay or co-insurance is due at the time of your visit. If you have not contacted your insurance company and/or are unsure of the amount of your co-pay or co-insurance, $25 is due at each visit.
